RADT-12. PRE- AND POST-OPERATIVE RADIATION TREATMENT PLANNING FOR PATIENTS RECEIVING STEREOTACTIC RADIOSURGERY FOR BRAIN METASTASES: A VOLUMETRIC ANALYSIS

چکیده

Abstract BACKGROUND Pre-operative stereotactic radiosurgery (SRS) has emerged as a recent treatment option to treat large or symptomatic brain metastases. Compared post-operative SRS, pre-operative may reduce rates of radiation necrosis (RN) and meningeal disease through treating smaller volume by preventing tumor seeding. We hypothesize that volumes will be than volumes, which in turn lead decreased morbidity. METHODS A retrospective analysis was conducted patients who had surgical resection SRS fractionated (FSRT) for posterior fossa metastasis were eligible inclusion. Both MRIs required allow accurate target delineation. added each patient, the clinical (CTV) used included. cavity compared using Wilcoxon signed rank test. RESULTS 28 received FSRT from 1/1/2016-12/31/2020 included this analysis. The mean 14.9 ccs, CTV 21.0 ccs (p < 0.01). 75% initial size planning. For with at least 4 follow up (n = 8), progressively 18.9 8.1, 7.1, 6.9, 6.2 ccs. CONCLUSIONS In study evaluating lower most patients. Previous studies including PROPS-BM have shown how risk RN because are used. metastases requires prospective validation.

Bevacizumab as a treatment for radiation necrosis of brain metastases post stereotactic radiosurgery.

BACKGROUND Cerebral radiation necrosis (RN) is a difficult to treat complication of stereotactic radiosurgery (SRS) that can result in progressive neurologic decline. Currently, steroids are the standard of care treatment for brain RN despite their adverse effect profile and limited efficacy. The purpose of this study was to evaluate the treatment efficacy of cerebral RN to bevacizumab in patie...

The volumetric response of brain metastases after stereotactic radiosurgery and its post-treatment implications.

BACKGROUND Changes in tumor volume are seen on magnetic resonance imaging within weeks after stereotactic radiosurgery (SRS), but it remains unclear what clinical outcomes early radiological changes portend. OBJECTIVE We hypothesized that rapid, early reduction in tumor volume post-SRS is associated with prolonged local control and favorable clinical outcome. METHODS A retrospective review ...

Stereotactic radiosurgery for brain metastases.

Worldwide, approximately 100,000 patients have undergone stereotactic radiosurgery for a variety of intracranial lesions, of which brain metastases represent the most common treatment indication. This article summarizes the major issues surrounding the management of brain metastases, and also analyzes 21 independent reports of Gamma Knife- or linear accelerator-based radiosurgery, representing ...

Stereotactic radiosurgery plus whole-brain radiation therapy vs stereotactic radiosurgery alone for treatment of brain metastases: a randomized controlled trial.

CONTEXT In patients with brain metastases, it is unclear whether adding up-front whole-brain radiation therapy (WBRT) to stereotactic radiosurgery (SRS) has beneficial effects on mortality or neurologic function compared with SRS alone. OBJECTIVE To determine if WBRT combined with SRS results in improvements in survival, brain tumor control, functional preservation rate, and frequency of neur...

HyperArc VMAT planning for single and multiple brain metastases stereotactic radiosurgery: a new treatment planning approach

PURPOSE The HyperArc VMAT (HA-VMAT) planning approach was newly developed to fulfill the demands of dose delivery for brain metastases stereotactic radiosurgery. We compared the dosimetric parameters of the HA-VMAT plan with those of the conventional VMAT (C-VMAT). MATERIAL AND METHODS For 23 patients (1-4 brain metastases), C-VMAT and HA-VMAT plans with a prescription dose of 20-24 Gy were r...
